The undersigned students, employees, and invitees of University of the Southwest, being 18 years of age or older, acknowledge that they have signed up for above named sport or activity, and that this activity is voluntary on their part and for which they will receive no compensation, either directly or indirectly, from University of the Southwest. The undersigned therefore assume all risk incident to the above listed activity, including travel to or from such place of activity and acknowledge that University of the Southwest, its Board of Regents, its employees and the State of New Mexico assume no liability, including financial responsibility for injuries or losses resulting from or occurring during the activity or travel to and from said activity. The undersigned further indemnify, release, and hold harmless University of the Southwest, their Board of Regents, their employees, and the State of New Mexico against any loss or injuries resulting or claimed to result from the undersigned students’/employees’ training, participation, attendance, travel or other involvement in any and all phases of the activity. The undersigned are aware University of the Southwest posts team rosters on the University of the Southwest’s web site, and therefore give University of the Southwest permission to post their names and photos on said web site. The undersigned further acknowledge that their signatures hereto evidence that they have read, fully understand, agree with and consent to the contents thereof.

UNSPORTSMANLIKE CONDUCT POLICY
Unsportsmanlike conduct will not be tolerated in the University of Southwest Intramural Program or during other University of the Southwest activities, and could result in ejection from games and/or leagues and events. A “bad-temper”, “natural-instinct”, “self-defense”, or similar excuse will not be accepted as a reason for misconduct. Unsportsmanlike conduct includes verbal and/or physical abuse of officials, profanity, fighting, pushing and verbal attacks aimed at opponents. The following disciplinary actions can be expected for offenses:
VERBAL ABUSIVE BEHAVIOR: If an individual is ejected from a game by an official or supervisor (whether after a written warning or on a first offense) he/she will be ineligible to participate in further competition or activities until he/she meets with the appropriate staff member.
PHYSICAL ABUSIVE BEHAVIOR: Physical abuse of an opponent, spectator, teammate or destruction of property will result in ejection from the game or activity and carry a minimum penalty of suspension for the remainder of the season and possibly semester. Physical abuse of an official or supervisor will carry a minimum penalty of suspension from all University of the Southwest Intramural Programs and activities for the remainder of the academic year. In addition to the aforementioned penalties, individuals who are physically abusive will be placed on permanent probation regarding intramural and other campus activities.
